I've tried a number of perk setups but none of them have worked

Warrior Mode + Glass Cannon got me until night 9 but didn't come close to clearing it

I’ve had a lot of luck with sword or spear, the war horse perk, and the damage reflection keep upgrade

Then you just wade into enemy armies and unleash hell

how do you deal with the huge waves of flyers if you're using a melee weapon

And you can actually be surprisingly effective anti air with the damage reflect by just soaking hits
