Every time I think I might use it instead of a I stop, wondering if it's supposed to be comforting or sarcastic as in "there, there" pats on the head.

Fang : yeah, Bless your heart means basically the person is defective someway and needs a blessing from god for support or allowances for their handicap. Used on a person not handicapped it's an insult.
