They're more like a little angel and a little devil quarreling. They're the positive thoughts and negative thoughts in one's mind.

The situation they mention in the poem is obviously dangerous. The situation involved diseases and death.

The person has a hard time deciding a better solution. At the end, the person still chooses to take the risk going out into that dangerous world and hopefully, the person will survive.
