also the medical history form starts with surgical history (which is empty, I'm a new patient) then asks "is this correct?"

ℙ𝕚𝕫𝕫𝕒𝕫𝕫 ℙ𝕚𝕫𝕫𝕒𝕫𝕫 : it was literally just "uses contraception" and then if I selected that, I had the option of a number of blood relatives. Couldn't say what the contraception was. Just ... that it existed
