What they wanted here was to do what Quinn did and split it into two independent clauses

anyway one of the evil witch's minions failed at his mission, so she phased through him, encasing his body in ice, then slit his throat just a little bit so that he'd bleed out slowly while frozen in place

i guess it is defined as a list of 3 or more but frankly a list of 2 should count too if you ask me

That's not a rule now but I can see the benefit of making it one

Not really that unnatural or confusing.

it's not necessary in all 2-item lists, but if your second item is multiple words long, you may want to consider a comma for clarity
