I've had characters die in games. I had one commit suicide to save the rest (but that was a) Rifts and 
the character was a samurai so that was totally in character for them)

the only time I've had a character die, it was friendly fire, he immediately got revived, and stabbed the character at fault in the foot. only their nebulous connection kept him from aiming for somewhere more... vital

having a cyborg samurai blocking a tunnel as his friends run down it, damaged to the point he's telling his internal computers to not shut down, and then as the enemy comes into view reciting a haiku about the sun and setting off his internal fusion reactor... it was a glorious death.
