Yeah, apparently a bad guy tried to blow up herself and Nanoha and while Nanoha survived, she lost her right arm and eye

Space magic utopia can probably regrow a limb or two, and I don't think the movies are supposed to be completely out of canon.

The commentary tracks are completely in character and feature the characters sitting around talking about it like they're watching a biographical movie.

So as far as I'm aware they're supposed to be somewhat dramatised versions of "real" events.
