I think based on how he reacted to his first canon kill with Doughy Tom, that he probably repressed it and that's where the Riddler half first showed up but got pushed aside so much that it wasn't until the "no body no crime" stuff happened that he had a chance to come out again
the psychotic break that brought about the second personality was just ... it felt like that personality was always there waiting for a chance to slip through a crack
ed had his quirks but he wasn't a murderer.. he was a guy with no understanding of boundaries and very much willing to frame others who had hurt him (see the M.E. who did a shit job) but overall, they showed no hints he'd be up for murder before that
Ed managed to drag an entire corpse into the GCPD in GIANT SUITCASES and no one went "....wait, why is ed bringing in giant suitcases?" gotham is so very gotham

wiki says that weird release kidnapping thing was three years after he was put in, so he's at least 18 at time of trial if he was put in at 15, potentially 20 or so
