Reasons so far today why I have not been able to enroll new subscribers:

1) You have to sign the last page of the form.

2) You have to write your address on the second page of the form.

3) You have to submit an application for the correct state.

that one I blame on the brokers: the members aren't downloading 2015 apps off our website, the brokers just have a stack of them in a drawer somewhere 'oh sure this one will be fine'
