Okay, during an early strike team log, the Doctor asked when exactly Altaïr was from, and he said late in the 12th century soon after the Kings' Crusade, which is another name for the Third Crusade.

And then Altaïr explicitly referenced Masyaf and the Assassins while he was hallucinating that the Doctor was Al Mualim.

Given that the wikipedia page for Masyaf mentions the fact that Masyaf castle was used by the Hashashin/Assassin order first thing,

it'd be pretty easy for the Doctor to put things together even if Altaïr hadn't specifically mentioned the Assassins.

And since he did, between these namedrops and the Doctor's knowledge of history, it was probably pretty simple for him to realize Altaïr meant Assassins and not just generic assassins.
